Infinity Hospice Care uses predictive data insights to improve end-of-life care

The number one priority for hospice providers should be to provide high quality care at every stage of life, but there is no time more crucial to uphold this standard than during a patient’s final days. CMS has brought more awareness to this need by implementing the Service Intensity Add-On (SIA). The goal of the SIA is clear: offset the high cost of intensive services and care so agencies can improve the quality of care provided at the end of life. But before you can schedule the appropriate skilled visits for a patient in their last seven days of life, you must first be able to accurately predict when those final days will occur. Too many hospices lack the predictive data required to anticipate patient outcomes.

Darren and Brian Bertram, leaders of Infinity Hospice Care, knew that using traditional methods to predict end-of-life would not be enough to achieve the level of care they were determined to provide. They turned to WellSky Hospice Applied Insights, and within three months of implementing the predictive data solution, they increased skilled visits in the last seven days of life by 97%.
