Understanding the 2020 Hospice Final Rule

The Centers for Medicare & Medicaid Services (CMS) have published the 2020 Hospice Final Rule, which includes a significant rebasing of hospice payment rates. Rates for higher acuity levels of care are increasing, while rates for routine home care are decreasing. Because routine home care represents about 98% of hospice days, this change will mean a reduction for many providers.

The 2020 final rule also includes a complex new requirement for an addendum to election statements. The addendum must provide details as to the care, services, and drugs that the hospice determines to be unrelated to the patient’s terminal illness and will not be covered under the Medicare hospice benefit.

Implementation of the election addendum has been delayed until October 2020, but other provisions of the 2020 hospice final rule go into effect on October 1, 2019, so your time to prepare is very brief. The time to begin is now.

Catherine Dehlin, RN, BSN, CHPN, CHCM is the Director of Hospice Services for Fazzi Associates. She is a senior leader in post-acute care having run home health and hospice agencies, including founding, growing, optimizing and ultimately selling a large hospice organization in Louisiana. In her role at Fazzi, Catherine serves providers across the country, focusing on educational development, optimizing organizational design, operational efficiency, advanced clinical modeling and quality improvement programs, and assisting organizations in achieving best practice clinical and financial performance. As a hospice and palliative care certified nurse and certified hospice case manager, Catherine reaches into the hospice philosophy by integrating a strong set of patient-centered core values and smart, sustainable business models.
